We are presenting to Medical Organizations, Nursing Home Groups and Home Care Agencies a medical platform which identifies those patients whose signs have recently degraded. These organizations are facing increased costs due to aging of the population and huge penalties in consequence of the Affordable Care Act. A gap currently exists between care and prevention, this gap is responsible for millions of unplanned hospitalizations and readmissions every year. Sentimed is presenting a solution built for managing large cohorts of beneficiaries and provide early detection of abnormalities in the patients’ health status, thus ensuring on-time course of action and reduced costs.
